Care Coordinator
Location: Telluride or Montrose, CO (Hybrid – 2 days/week in office)

JOB SUMMARY
The Medicaid Care Coordinator provides person-centered, community-based care coordination for individuals enrolled in Medicaid. This position supports clients by conducting outreach, completing assessments, reducing social drivers of health (SDOH) barriers, and connecting individuals to essential services such as medical care, behavioral health, housing, food resources, transportation, and benefits.
The Care Coordinator conducts both phone and in-home visits, schedules follow-ups, coordinates with providers, and ensures accurate, timely documentation. The role strengthens relationships with community partners, increases awareness of TCHNetwork’s services, and supports program workflows and quality assurance expectations set by the Care Coordination Supervisor and Manager.
Compensation And Benefits:
The starting salary range is $21.00– $ 23.00 per hour, depending on experience.

Community Programs Manager

JOB SUMMARY
TThe Community Programs Manager provides leadership, oversight, and operational support to Community Programs Navigator Leads and frontline staff delivering enrollment, outreach, and resource navigation services. This role ensures high-quality, consistent service delivery across programs including SNAP outreach, Energy Outreach Colorado (EOC), Connect for Health Colorado (C4), Division of Insurance education, and other community-based initiatives.
The Manager supports staff through coaching, workflow development, documentation oversight, reporting, and partner coordination. This role also leads outreach strategy, supports compliance with state and funder requirements, and collaborates with internal teams to ensure programs are accessible, effective, and aligned with organizational goals.
Compensation And Benefits:
Salary based on experience $60,000 – $65,000

Quality Improvement (QI) Coordinator

JOB SUMMARY
The Quality Improvement (QI) Coordinator supports high-quality, consistent service delivery across Care Coordination and Community Health Worker (CHW) programs. This role focuses on improving documentation, strengthening workflows, completing quality audits, and supporting TCHNetwork in meeting new performance measures under state and regional contracts.
The QI Coordinator assists in developing workflows, conducting QA reviews, supporting new hire training, completing reporting for grants and funders, and identifying areas for improvement. This role does not supervise staff but works closely with the Care Coordination Supervisor, Care Coordination Manager, and Co-Executive Director of Community Programs to support training, workflow adherence, and documentation consistency across the team.
Compensation And Benefits:
Starting salary range is $23.00 – $25.00 hourly, depending on experience.

This position supervises a small team and ensures staff are supported, trained, and accountable for meeting documentation standards, workflow expectations, and performance measures tied to state, regional, and grant-funded contracts.
The QI Supervisor balances people leadership with hands-on quality improvement work, including audits, training, workflow refinement, and reporting support. This role works closely with the Care Coordination Manager and program leadership to strengthen systems, reduce documentation gaps, and build staff confidence and competency in daily workflows.

Starting salary range is $28.00 – $31.00 hourly, depending on experience.
